The approved policy should explain who may report, the types of concern covered, confidential handling, investigation, protection from retaliation and external routes where appropriate.
Responsibilities
Clear ownership and escalation.
Leaders must promote a speak-up culture. Managers receiving concerns must protect confidentiality, avoid conflicts of interest and refer the matter to an authorised decision-maker.
Reporting and review
Concerns must be acted upon.
The final policy should identify confidential internal contacts, alternatives where normal management is implicated and appropriate prescribed external reporting routes.
Review dates, document ownership and the approved full policy should be confirmed before publication.
